Liquid Kids on the Saturn with spinecard, an insanely priced game, but near and dear to me.
Also a loose Bubble Bobble Part 2 NES cart, not as rare as LK, but still.

Excellent game! and the Saturn port is tops!
The Taito Legends version glitches out on my PS2.
I recall back around 1999 when I first got a computer and discovered NCSX.com, they were clearing out copies of Liquid Kids for $30. ...I had no interest in colorful 2D games and scoffed at the notion of wasting money on such a game.
Maybe 5 or 6 years later I became familiar with Liquid Kids and wanted to own a copy. It took years of monitoring eBay before I was able to buy a copy for less than $100.
One of my rarest items would have to be the "Rez Perfect Intoxication Set"
Unfortunately the vibrating headphones don't vibrate any longer.
Lik-Sang was literally giving these away, brand new, back in '05. I got one when I bought a modded Dreamcast from them.
Uh, let's see what I could list here:
CIB Panzer Dragoon Saga
CIB X-Com UFO Defense PS1
CIB Terranigma UK
CIB Doremi Fantasy
CIB Downtown Nekketsu Monogatari PC Engine
CIB Parasol Stars JP
There may be more; I'm just going off the top of my head right now

Since you said item and not game, it would probably have to be my Jet Set Radio Beat Prototype figure since the last one I saw selling went for $200
Unopened Ninja Gaiden 3 on NES
What do you mean I have no life?? I'm a gamer! I have lots of lives!
Quartet for the Sega Master system. Probably not that rare, but the rarest I own. That and a copy of Sonic Spinball since I'm pretty sure anyone else would have returned it or destroyed it for being such a horrible game.
2 Biohazard 5th anniversary boxes
My Systems: Atari 2600, 7800 Intellivision Nes(top loader) Snes Virtual Boy N64 Gamecube Wii GBC GBA(original) DS(brick) Genesis Dreamcast Ps2 Ps3 PSP(phat) PSP 3000, LYNX
i liked sonic spinball too
my cousin had it and when i was younger would goto my aunts house and play that game!
i have it now on my sonic collection disc for the xbox.
The problem with it that i feel gamers had is it seemed so rough.
The animation seemed very rough (like when you make sonic curl up and charge up for a spin attack the animation seemed i guess the best way to describe it is blocky) and the audio was kinda bad even for sega genesis. The controls seemed kinda glitchy on some of the levels (the second one where you gotta propel sonic up the shoot with the air cannons by pushing the action button... you had to time that thing pitch perfect or else it wouldn't work which frustrated gamers like myself)
If you can get past those things... Its actually a really fun game!
my rarest.. god i don't even know now that i have this crazy amount of games i bought in the past 5 months.
i'd have to go through them and look em up and everything.

SEGASATURN HST-0004 Developer Version
SEGASATURN CartDev Rev.B
Don't have much rare stuff, I only collect games I really like but it's probably a pretty minty Crisis Force CIB.
SEGASATURN Pre-production Prototype
Very cool Saturn DJ. I don't have anything ridiculously hard to find, but I would say mine are:
Panzer Dragoon Saga (complete)
Earthbound (cart only)
Phantasy Star for SMS(complete)
Complete in box Virtual Boy
Probably a sealed Daytona Netlink
Diabetes Sucks!
Mmm... I would say Pocky and Rocky 2 for the SNES, a Sealed Kirby's Adventure/ Loose Romance of three kingdoms II / Chip n Dale/ a complete Alien Vs predator for Jaguar and International Superstar Soccer for SNES too.
I have a lot of SNES/NES Stuff, but I don't know how rare they are, because the rarity here goes very diferent from what I see here!
Probably my sealed copy of Zelda's Adventure for the CD-i.
a few pretties and a sealed Earthworm jim
Either my CIB copy of Snatcher for Sega CD or my copy of Pier Solar: Reprint Edition. I have a feeling that there are a lot less Pier Solars than Snatchers in the world, so Pier Solar is probably my rarest.
Legend of Almana and Doki Doki Panic pirate carts!!!!!
