I haven't been collecting too long, but there you go.
My 2 rarest games are my CIB Bubble Bath Babes for the Nes and the other is probably my U.S. Rage of the Dragons for the NEO GEO AES. I think only like 200 U.S. versions were produced but I'm not sure.
Music Machine for the 2600. Not only my rarest, but probably the cheapest I paid for any single piece in my collection........ 25 cents at a flea market

Real collectors drive Hondas, Toyotas, Chevys, Fords, etc... not Rolls Royces.

My new rarest item is Muncher for Bally Astrocade. R8 I believe, sure it's a tie with some RCA Studio II junk I have, but what is more desirebale is worth more. So I say Muncher.
My rarest is probably a CIB SNES. I bought it back on the original release date and have kept it in my basement ever since. One of these days I might sell it.
My Atari Jaguar Proto CD Set is the rarest,I guess.Supposedly,they're the only ones in existance,I'm told. They belonged to a former Atari Corp. employee.
Just noticed that GameCube Action Replays are worth quite a bit. I have a CIB Action Replay Limited Edition w/ 64 MB Memory Card. There's a completed CIB listing on ebay that went for $215. So that's probably my most valuable.
Also, I hear that the PSX Analog controller (not Dualshock) is rare. I have one of those too.

I used to have the Earthbound scratch and sniff pizza delivery guy hang-up thing that you got for sending in a questionnaire, I forget if it was in Nintendo Power or the back of the Players Guide that came with it.
I seem to have lost it sometime in the past 10 years though.

According to NintendoAge, my rarest Nintendo item is the box for Bandit Kings of Ancient China. I had two CIB a couple years ago, and managed to find a bidder at auction for the extra on Ebay for about $25. They might be worth more now. I also have the box for Arkanoid. I don't know my rarest Playstation, Atari or Genesis items.
Well here's some widely known semi-rare Sega MegaDrive/Genesis pirates I picked up months ago.
Most of my rare games aren't super rare, just higher demand than supply, like Panzer Dragoon Saga and Suikoden 2.
Equipment, about the only thing I have that'd be terribly rare would be the Action Replay for the Genesis. Had to send away for it from a magazine ad and then they came out with a newer version that didn't use the same codes or something so it wasn't very useful in the long run. Of course if I'd just waited I could've gone to any store in America and bought a Game Genie instead, but I'm impatient, lol. I even went to Canada to buy a NES Game Genie back when they were banned in the US.
I only know one other person who has it. I know next to nothing about it. I'd like to know how it was circulated.
EDIT: Doing a little searching, I read where someone said they mailed in some candy wrappers for a Lost World game or demo (forget which they said) but got SSv2 instead.
