mine would be:
intro - Soul Blade (PS) i LOVE that song, it fits so well to the intro WOW
ending - i really have no idea , i like almost all rpg endings

Best intros:
Virtual On: Cyber Troopers (Saturn) - the whole is just really cool and makes you want to play the game!
Crazy Taxi (DC/GCN/PS2/Arcade) - that Offspring music comes blasting through and the taxi's start flying! "YEAH YEAH YEAH YEAH YEAH!!!!"
Super Smash Bros. Melee (GCN) - a very slick, high quality FMV is excellent, showing all the characters in various scenes.
Gran Turismo (PSX) - the whole starts off really subtle and quiet and the sounds of finely tuned engines roar and that GT logo comes in....oooh yeah!
Soul Calibur (DC) - the whole intro segement is long and well constructed, very awe inspiring.
Starwing/StarFox (SNES) - despite the limited graphics, the dramatic scenes are well depicted and that title screen music is quite forceful.
Stunt Race FX (SNES) - really gives you a good feel to the game and provides some catchy music to boot.
Wave Race 64 (N64) - even for such an early N64 title, this intro scene still manages to show off what the N64 is capable of.
Lylat Wars/StarFox 64 (N64) - breathtaking graphics and music make this a truly cinematic experience.
Best endings:
Lylat Wars/StarFox 64: Really, this is one of the few games I've played where the ending sequence really made the whole thing worthwhile.
Most game endings are a huge let down after such a great game (e.g. Ocarina of Time, Super Mario 64)
Super Mario RPG's intro was one of the highlights of the game.
As for endings, well, I really liked Chrono Trigger's endings. Not only was the music great, but they provided a decent sense of closure that many other games lack.

Holy crap. It's been a while.
I've always enjoyed the intro for Crystalis for some reason and as for endings, I can't pick just one right now, their are too many that I like

FF7 had a AWFULL ending. Now 8 and 9, on the other hands -- they explained everything, went on for 30 minutes, and brought a tear to my eye.
Best intro? Oh geesh.....after all the games I've played, there's just too many of them. FF8 comes to mind again, as does Ninja Gaiden 2. Xenosaga's is pretty good.

I'm seconding Sonic Adventure's opening, I still love it.
Super metroid's opening was cool too, with the actual voices.
Endings, offhand Lunar 1 and 2 almost had me in tears they were so sweet.

FF7 is the ONLY modern day RPG with FMV that has really impressed me with Intro and Ending. FF8 was a turd in my book and 9 was good but just didn't blow me away the way 7 did.
Other than that, most games intros and endings are fairly forgettable to me, its the game I'm interested in.
SO not the Drama.

However, my favorite intro back in the day when you couldn't do those kind of things was the intro to NES Ninja Gaiden.. I dug how those guys jumped up and clashed swords.. both land, but only one stays standing. Cool stuff!
My favorite ending.. hmmm. Tough one. I guess I dug the ending to Ghouls and Ghosts on the Genesis.. where it showed you doing different action sequences.. I dunno. Yeah.. poor choice. But I forget

Man, this is a tough topic. As for opening scenes, Valkyrie Profile's hit me harder than I ever expected (though Platina's actress is incredibly annoying). I also liked the introductory animation for Xenogears, which doesn't even seem to connect with the game until you're 50 hours into it. That was a clever tease.
Of course, there's something to be said for the music-video school of game openings, and here I prefer the amazing footage from Chrono Cross. Soul Blade is also impressive, and it still comes off as cooler than Soul Calibur's intro.
Endings, endings . . . Well, I've always enjoyed the Final Fantasy series, but I especially liked Final Fantasy X's bittersweet finale, even if it seems to be undone by FFX-2.
I also dug the shocker at the end of Final Fantasy Tactics, the unorthodox resolution of Xenogears, the stylish denouement of Vagrant Story, and that wonderful sequence at Terranigma's close. Panzer Dragoon Orta had a nice ending too, despite the fact that it didn't make much sense.
And nothing beats the Ninja Gaiden II ending. How can the sword DO that? This is a Nintendo game. That's how.
