The first game I recall having profanity in it is Final Fantasy VII, courtesy of Barrett. The first game I heard with spoken profanity is Kingpin: Life Of Crime on the PC. The first game that implied profanity, of course, is Q*bert. Not sure which game was the first to feature either of the two most popular 4-letter words.

The SNES version of FF6 had one line of censored swearing.
Breath of Fire II had one smart-ass fly boss that said "Damn!" when you beat it. Only cussing I've seen in a Super Nintendo game. (I think Conker was the first game that Nintendo officially allowed swearing?)
It wasn't in game, but the NES Metal Gear had the infamous "FUCKY OURMO MMAAN DYOUR DADDY" password.
Maybe the first Nintendo published game, but not the first on a Nintendo system. WWF games on the N64 certainly used the word ass, and I'd be surprised if those were the first after game ratings started and Nintendo let up on their own standards.

What about “Oh Shit” by Aackosoft, from 1985? It was on the MSX computers in Europe. It was a Pac Man clone but when the character got eaten by a ghost it would say the titular line.

Yeah it looks like people above have found some really old games that included it.
But I'll tell you the examples I have thought of anyway.
(1995) In Gex for 3DO/Playstation/Saturn USA version, one of his lines was "who opened the can of whoop-ass?"
(1997) In Vandal-Hearts for Playstation there are multiple instances. Diego says "Shit!" and Darius says "They look pissed!" which IMO isn't true cussing but some parents might not be ok with hearing their kids repeat it. Ash says "Kane, you bastard!" "Damn" is also used many times when characters are informed there is nothing they can do about a situation at the moment.
(1997) In Courier Crisis for Playstation/Saturn, you can flip off drivers who will get angry and honk. If you flip off a police car, they will arrest you.
(2000) Perfect Dark predates Conker's Bad Fur Day mentioned above by almost a whole year. Sometimes when you shoot and kill a guy they yell "you bitch!" as they die.
