The old school game that gets the most replay around these parts is probably Virtual Pro Wrestling 2. That game will never get old. It's perfect. X-Men vs. Street Fighter comes in second. After that comes other fighters and various party games. I guess multiplayer games just get more play, even if I like some single player games more. Gaming is more a social thing around my parts.

I'd have to say Tetris on the SNES Tetris/Dr. Mario combo cartridge is my all time favorite version of Tetris ... and the one old school game I've been playing for years since its release. It's truly timeless. There's something about this version that makes it superior over all other versions. I think it has to do with the presentation ... the SNES has a marvelous sound chip that gives the three Gameboy original Tetris tunes that extra robust flavor. Couple this with the vibrant SNES graphics and the 2 player competitive mode (missing from both NES versions), and you get a version of Tetris that endures forever!

Runner up: Tetris Attack on SNES (I prefer this version to Pokemon Puzzle League on N64 or Planet Puzzle League on the DS)

    Dreamcast:
Sonic Adventure 2. I've played thru this game soooo many times i've pretty much got it memorized. The best of the 3D Sonic games IMHO.

    PS2:
Persona 4. Reallllly long RPG and one of my favorites from the Persona/Shin Megami Tensei franchise that features some light dating-sim elements. Reallllllly good.

    Super Famicom:
Super Famicom Wars. The second incarnations of the Nintendo "Wars" series, and realllly freaking addictive. If you like Advance Wars on the GBA, (and have a way to play Super Famicom games) you should definitely check this out.

    N64:
Custom Robo. One of my favorite 3-D mecha games (easily up there with Virtual-On and Armored Core) and what I would say is the really good mix of the two. You can customize your mecha with different weapons and then fight in pretty awesome 3D arenas that easily surpass the Saturn version of Virtual-On. (FYI, the N64 version is import only)

    Saturn:
Dragon Force. I absolutely love Strategy RPGs and Working Designs made some of the best ones back in the day (The Saturn version of Magic Knight Rayearth was a pretty awesome RPG too). You take control of massive armies and conquer several kingdoms all over the world, and it's one of the best S-RPGs from the time period (1997?) and IMHO, would easily beat out Final Fantasy Tactics on the PS1.
    ==SuperFamicast64's Console Collection==

Dreamcast, PS2, Super Famicom, N64, Saturn, PS1, PSP, GBA, Genesis (w/Model 2 Sega CD), WonderSwan, SwanCrystal

I don't think there's any single game that I replay frequently. If you asked me in my youth, then, yeah, I replayed some games a ton because my collection was small, but now I have too many unplayed games I want to get to. That's not to say I never replay anything, but when I look at a single game, even if I look at my favorites, we're still only talking a replay once every couple years or so. And that's only because those games, like Tail Concerto, Wonder Project J2, Dragon Spirit, etc., are short. There are other games I like even more, but if we're talking a 40+ hour commitment, then it's going to be more years yet before I bother with a replay.

I don't fire up my retro system nearly as much as I use to, and it's usually random games for most, but there are a few that are staples.

When ever I fire up my SNES I always end up playing a few games of Firepower2000. I also always play at least one game of Mystic Defender whenever I play my Genesis, and for Saturn it's always a few games of Bust-A-Move 2.

The best, that I still play, of all time isn't easy for me to decide, and I don't want to list a top two or top ten since that isn't what the OP asks for.

NES Ice Hockey is one of the best vintage games for replay value, and I have been playing it often although not all of the time, but I do not consider it the best of all time.

Perfect Dark on the N64. I love that game so much.

Honourable mention goes to Galaga '90 for the PC Engine. I've never owned the real hardware but I've played that game on pretty much every emulator out there.

I wish I could afford to get into the PCE. It's such a great looking machine...

Does Super Star Wars for the SNES count? How about Time Pilot on Konami Arcade Classics?

Hmmm seeing how I play nothing but classic games lol...

    Arcade - Twin Cobra
    Atari 2600 - Pitfall 2
Colecovision - Wargames / Time Pilot / Turbo
    Intellivision - Snafu
    NES - Contra / Zelda
    Genesis - Earthworm Jim
    Amiga 500 - Agony / Project X / Speedball 2 Brutal Deluxe
Amiga 1200 AGA - Banshee / James Pond 2 Robocod
    Jaguar - Tempest 2000
    Dreamcast - Crazy Taxi

I have a lot of other systems but just don't touch up on them all that often unless I'm really bored.

Ones I keep hitting again and again:

    Atari 2600:
    Cosmic Ark
    Raider of the Lost Ark
    Berserk

    SMS:
    Space Harrier
    Double Dragon
    Zaxxon 3D

    NES:
    Nobunaga's Ambition
    Metroid
    Solomon's Key

There's lots of games on the Colecovision that I love to play, but until I get one of them action controllers, I can't give it more than an hour without losing all feeling in my arm.

I swear, I've play through Mega Man X at least 3-4 times a year ever since I was about 8 years old. Love that game.

Hands down, Phantasy Star Online for the Dreamcast, played offline of course. I spent a lot of time online when it was current, and all these years later, my quest to find a legit spread needle continues. Two days ago I began yet another character build on a fresh VMU.
