Has anyone ever seen a CBS Colecovision cart with a label like this? It has a black & white canadian cart label but it has been trimmed on both sides to fit the CBS cartridge. I included a pic with an american cart for reference.

Chances are the Canadian label was simply affixed on the European CBS Colecovision cart.
Now here's another one... albeit in very poor condition.
It would seem that there is nothing wrong with this Colecovision cart label. Untill you notice that it is affixed below the molding on the cart where a "normal" Coleco cart label would end. But why, I ask does it have two end labels on the top? It doesn't seem like they just cut it too long and stuck it on there. There wouldn't be another end label on top of the other one if that was the case.
This is a French Canadian cart -to the extent that all it says is COLECOVISION presente Authentique ZAXXON de SEGA.
What's with these Coleco carts?
Any ideas?

It seems to be another case of an English label that had a French label put on top of it, or rather mis-applied. Notice that the label on the end has "TM", whereas the French label reads "MD".
At the very end of the Coleco's lifespan I can recall Toys R Us in Montreal was selling CV carts in blister packs with plain white labels and simple black lettering. I never did get to the bottom of why.
I never knew these carts actually had a Canadian address on them. Makes me wonder if there's someone in Montreal with heretofore untold knowledge of Colecovision history and a bucket of prototypes or something.
