That's just an old Space Invaders cocktail with probably just a common bootleg Puckman in it. That cocktail cabinet was everywhere where I grew up and it wasn't uncommon seeing a bootleg Puckman in it in later days :P Matter o' fact Taito just released a commerative controller for their Space Invader 25th anniversary which is a mini version of that particular cocktail.

I also remember the maze getting invisible in later stages.. although I don't specifically remember seeing it with hearts and the 5 exits.. But I could be mistaken. We had al sorts of Puck boots and I hated even looking at them let alone playing them
