Let's just say this... These are the games my wife won't let me play if she is home.

Friday the 13th on the NES was damn scary, considering that Jason often popped out of nowhere when you least expected it. No matter where you were, on the path, on the water or in a house. It may seem stupid today but let someone who has never played it before try it for the first time in the dark. The thing that rattled my nerves the worst was that every once in a great while, from what I recollect, he could show up in a house even when his energy bar wasn't on the screen (which usually indicated he was inside). This was usually if you actually pursued him on the path, or he pursued you, and he was in between stops with the kids or counselors.
Sure, Friday the 13th doesn't scare quite like Resident Evil or Silent Hill but the way the music suddenly changed and Jason just appeared on the screen to attack you still gets to me once in awhile.

For me, it'll always be Clock Tower: The First Fear.
I don't see how people can possibly get scared in any game that features heavy weaponary. Surprised maybe, but a real horror fan knows.

I second Friday the 13th for NES. That game is scarier than RE. Slient Hill2 is pretty freaky.
I really should have played more of that when I rented it.
As for requirements, I'm more a fan of things with a creepy atmosphere or ambiance as opposed to those that rely more on cheap 'jump' scares over and over again, more of a 'Lovecraftian' approach, if you will. That said, I remember jumping pretty excitedly when the first of the zombie dogs came crashing through the windows in that lonely hallway in the first Resident Evil. I must say, the whole "evil children" (especially the singular long-dark-haired female variety) gimmick has worn itself completely and thoroughly out. In what little I played of FEAR, I just sort of shook my head whenever a 'spooky kid' was on-screen. Time for a new cliche.
Don't know about the scariest video game of all time, but as a kid I was scared of that Demon looking head that said "Beware, I LIVE!" in Sinistar. I dunno for some odd reason that always unnerved me, seems stupid now but back then I hated it. Made my hair stand on end. CLOCK TOWER on the Saturn (iirc) was pretty fucked up IMO.
When Eternal Darkness told me it was deleting my saved games, it scared the Hell out of me.
I thought Doom 3 had some scary moments in it.
My dad raised me on monster movies & Sci Fi (in the 60's kids.. ). Therefore.. I grew up totally desensitized to horror and being scared. To this day.. there's only one movie that ever made me "jump", and up until recently.. no game. But I broke down last year and finally bought Silent Hill 2. I CAN'T finish that game.. 'nuff said.
I DO think Pyramid Head is the coolest guy since Pinhead in the Hellraiser movies though.
How could we, on a retro-gaming site, forget a game like Project Firestart? If you haven't played it already, then do yourself a favor and grab a C64 emulator and this game. You'll be switching disks relatively often as the game is huge, but the atmosphere this game creates, especially considering the hardware running it, is astounding. Some jump scares here and there I admit, but at points it really will have you second guessing whether or not you want to open that next door, even if you do have a laser rifle in hand. I suggest playing it night, by yourself, for the best effect.

Some of the insanity effects in that game are pretty neat. I'm still hoping for a sequel to that game.
Silent Hill (PSX) is probably the scariest one I've played, although there were a couple of areas in Killer 7 (GCN) that had me jumping.
Kuon for PS2 has a rather creepy ambiance - there's nothing like old-school Japanese historical horror games.

I often got scared playing Tomb Raider when I was a kid because the traps would spring out of nowhere and kill me.
