The copy I've bought from 4th Survivor is the only one I know with 2 booklets and the letter you had to submit to partecipate.
As for the booklets, one is the same that comes with Donkey Kong Competiton carts, the other is completely different.

I've seen the cart before (back in the 90s when competing in the competition) however have never seen the booklet.. any chance of posting a screenshot?

Edit: Last night I remember going through my basement looking for the Blockbuster World Video Game Championship picture frame with my name on it and found it. I'll have to post it later when I get home from work.
