This is probably the most difficult 2D Mario game. I've beaten it about 3 times.
Yes, 8 times through and also through World D
Yes, world 1 through world 8
Yes, but with warps
No, it's too hard

Twice; once on the SNES going through all the levels, and once on GBC going through all the levels.

Infinite Lives
I've beaten it three times on Virtual Console and twice on SNES. It's more difficult on VC/FDS because you don't restart on the current level you're on, just on X-1.

Anyone beat Super Mario Brothers 2: the Lost levels
There are some excellent answers in the [similarly titled] thread from last year.
I haven't yet. It is pretty darn hard. I think I've only gotten to world 3...

I plodded through it on All-Stars. Saving EVERY level, and dying many many times on certain levels.
A few years ago, I tried to see how far I could go without continuing, but I don't find games to be enjoyable when they are THAT hard.
I turned it into a poll right when I made the thread. Not my fault that it lets people post in between the time I make a thread and the time the poll is done. I'm not sure about your level, but people like me can't change a thread into a poll after the thread is already posted. I'm not the type to pull a fast one >_>

Never have beaten it. Too hard for me.
DERP
I've beaten it through a few times, finished D too, pretty hard but not impossible.
If they made a movie out of your top five worst sins, what would it be rated?

I've done it a couple times, and at least once without warps.
It usually takes me awhile to get through that level where you have to go platform-to-platform via green trampolines (pretty sure it's World 6 or 7). I think there's wind also. God I hate that level.
Surprised to see anyone else that has played the letter worlds here, or did they make them easier to play in Super Mario All-Stars? I always ignored the Lost Levels in the SuperNes game and only really tried playing it when it saw a Virtual Console release.

Yeah I beat the game on SNES plenty of times. I can get through world D-4 on the SNES version, including A-1 thru D-4.
I've beaten the GBC version only once because I'd prefer to play either of the console versions. I don't like that screen scrolling on the GBC very much.
My favorite version is the original 8-bit FDS version. I've done it twice on Wii VC and four times on the NES repro cart I had made for me. The nice thing about the original FDS version is that you have unlimited continues even though you have to start on level X-1. X being whatever world you were at.
The level that trips me up the most is the cloud level, world 8-3. Those hammer bros. can be tricky to dodge unless you remember which ones jump and which ones stay on the ground when you run at full speed. After a few tries you can memorize their attack pattern. World 7-3, the windy springboard level doesn't give me much trouble but it used to when I was a kid.
The cool thing is if you beat the game without taking any warp pipes. In the FDS and SNES versions, this allows you access to world 9. It's completely underwater except for 9-3 which is a strange outdoor castle. World 9 comes between 8 and A in the SNES version. If you warp and save the game on your SNES file, you can't get world 9 unless you start a new file again. The FDS version of world 9 only starts you out with 1 life.
I never knew you could get to worlds A-D on the FDS version. I should do a full playthrough so I can do it on video. 8 times is a long time to play an NES cartridge repro but it should be fun.
I 've actually never beaten the game WITH warps. The first time I played it on the SNES it tookl me 5 hours, 2 minutes, according to my log I keep on game I've beaten.
On the FDS, it took me two or three tries, making it all the way to 8-4 and losing all my lives when I was so close before I finally nailed it. However, I don't recall havong to go through 8 times to get to the letter worlds. I think they were just there when I beat world 9. I forget now. Crap.

The moral is, don't **** with Uncle Tim when he's been drinking!
So there are no other reasons for having not beaten the game other than "it was too hard"?
Many times over on Super Mario All-Stars. It's definitely a conniving, malicious game, but I never understood the extent to which people complain about it being too hard.
