Rarest thing I ever had was a Nintendo 64 test cartridge but I sold it a few months ago. I guess the most rare item right now is the official Sega power strip I bought the other week.
Could be my Hori Mini N64 controller. Or R9 rated X-Man Atari VCS game. Maybe my recent purchase of D2K Arcade for Intellivision?

In terms of games, probably my Zenki for PC-FX. I have a lot of obscure game-related items from Japan, though, some of which probably have even smaller print runs.
Among possible choices here;
1) Locked copy of Actraiser in a Satellaview 8M Memory Pack.
2) All Night Nippon Super Mario Bros./Super Mario Bros. 2 (J) FDS disk.
Almost all of mine are super common too.. I guess the rarest might be my recently acquired (thanks to a generous friend) copy of X-Com: UFO Defense for Playstation 1.
Probably my 3 cib snes store displays for starfox, Mario mart, and Mario paint. Used to be snatcher before I traded it.
A cart only copy of Cheetahmen II.
Probably my SCPH-5903 mint PSX.
Panzer Dragoon Saga for the Saturn. I had an opportunity to purchase several other copies from Toys R' Us, when they were discounted to $20, but there was no such thing as Ebay back then.
My second rare would be the Super 8 for the SNES.
Nothing truly rare, but the most uncommon ones would probably be CIB Gourmet Sentai Barayarou for the Super Famicom, Rockman Megawars for the japanese Megadrive and Keio for the Mega CD.
My rarest item would be my complete The Amazing Spider-Man: Web of Fire for the 32X. I actually bought it new when the game originally came out. I guess it's a good thing I held on to it.
