Well for questions 1 and 3 it hard to say. The collecting world has been turned upside down since these reprints started showing up. To put value on what use to be rare game's psx cd's is very hard now some people (collectors)in the know are just sitting and digesting this new issue.
I'm am truly starting to think that there may never be a rare game cd ever again.

There's always going to be rare game cds because the demand for some games will never be great enough to warrant additional print runs. Some games just flat out suck and it's doubtful that the supply will ever be less than the demand from the hardcore collectors (people that insist on having every game released in a region).Originally Posted by gepeto

As far as rarest game, I've been working in a game store for several years and been going around collecting for just as long and there are just a couple that I have never seen in the wild. Not including Lightspan games or NBA 2ball the only ones I could never find were Fox Hunt (bought it on EBay), Psychic Force and Psychic Detective. X-Com was on my list too until a couple weeks ago I stumbled upon one at an EB store for like $6 I think.
But then again thats just my experience, nothing scientific.

1. Rarest individual games arn't actually the games that pull in the money. If I told you Easter Bunny's Big Day you'd laugh and spend like the 5-15 it'll run you, and scoff at the $160 Suikoden 2 and the other big money/demand games would run you. @ 8-bitNesMan. There is an actual small Jewel case release. DB's remembered seeing it once before he was finally able to stumble into the manual missing copy that he currently has. Yours is the regular one.

I think I have seen the jewel case MK3 once (didnt realize it was as rare as it sounds like it is). You just can't go flat out make a comment like "___ is the rarest PS game". The rarest, by definition, will be something that was a small time release, probably very terrible in all aspects, and almost nobody remembers, hence has fallen into obscurity.

I think it's perfectly fair to say "X" is the rarest game. It's simple, the game with the smallest printing is the rarest.
However, as we all know rarity is not value. That's why FFVII still fetches $15+ while Lightspan discs gan be had for <$10.
It doesn't get any more serious than a Rhinocerus about to charge your ass.

Actually, sales data for PSX games was floating around for a while. The problem is that there are a couple dozen titles with sellthrough under 7500, and the data isn't accurate enough to use that as a guide.
Aside from variants, LS and demos, I still think Easter Bunny's is the rarest. I can't think of a single regular PSX release, Rumbo a la Copa Mundial included, that I haven't come across in the last couple years in the wild though.
