My rarest was probably my complete Titlematch Pro Wrestling for Atari 7800 in the big box but I sold it. Next in line would probably be...
my sealed Buster Douglas boxing for sms
or my rarest 2600 games
Waterworld
Crazy Climber
Rescue Terra I
Nothing really rare, but here's a few I have that are not common in this condition:
Castlevania Sotn black label factory sealed
Castlevania Chronicles factory sealed
Castlevania Adventure factory sealed

Dark Wizard (sega cd) factory sealed (kind of hard to find sealed copies of sega cd games; especially this one).
my rarest would probably have to be...
Sega 32X: Spider-man: Web of Fire
soon it will be something better hopefully.
Two of my rarest cartridge games would have to be some Commodore Vic-20 ones:
1) Frogman by Machine Language Games
2) Fun With Music by Epyx (which I cannot seem to find listed in the Digital Press Online guide)
My rarest item would have to be castle hassle for atari xe Complete boxed. or possably stadium events ntsc-usa but its not boxed i know it worth more but not sure if its more of a rarity.

Great thread.
I can't choose one, so I'll list my guesses:
VS Castlevania complete conversion kit
Playchoice 10 Castlevania daughterboard
Castlevania (Amiga)
Castlevania Lament of Innocence: Special Edition(not to be confused with Limited Edition)
Haunted Castle (arcade)
Akumajo Dracula (arcade, japanese haunted castle)
Dracula X: Nocturne in the Moonlight official hat and jacket
Less rare items include Vampire Killer Megadrive, Dracula X: Rondo of Blood, etc, but these items despite their high cost, are far more pedestrian rarity wise than the listed items.
I don't have a whole lot of really rare stuff. I'm not too hardcore a collector. But here's what I have...
Body Slam Super Pro Wrestling (Intellivision) R7
Diner (Intellivision) R7
Stadium Mud Buggies (Intellivision) R7
A.I.R.C.A.R.S. (Jaguar) R8
Bignose Freaks Out (NES) R7
Quattro Adventure (NES) R7
Iron Soldier III (technically it's the full game with 'demo' written on it) (Nuon) R7
Keio Flying Squadron (SegaCD) R6
Bonk 3 (TurboGrafx-16) R8
My rarest item is easily my Daewoo Zemmix (CPC-51B). It took seemingly forever to find!
It'd probably be something on the Atari Jaguar. Either the unreleased Spacewar 2000 game or Charles Barkley Basketball, both of which I ordered online last year.
Playstation:
Discworld (R5)
Herc's Adventures (R7)
Psychic Detective (R8)
Titan A.E. demo disc
Saturn:
Lunacy (R5)
I do own a limited collectors RE4 art cell shrinkwrapped from the Gamestop limited edition, i dont know if that counts.
my R6 games...
SNES
-----
Aero the Acrobat 2
Sim-Ant
NES
----
Mermaids of Atlantis (with box)
PS1
----
Klonoa (no case)
Nothing remarkable. But I did get them out of the wild
