According to NintendoAge, my rarest Nintendo item is the box for Bandit Kings of Ancient China. I had two CIB a couple years ago, and managed to find a bidder at auction for the extra on Ebay for about $25. They might be worth more now. I also have the box for Arkanoid. I don't know my rarest Playstation, Atari or Genesis items.

Most of my rare games aren't super rare, just higher demand than supply, like Panzer Dragoon Saga and Suikoden 2.
Equipment, about the only thing I have that'd be terribly rare would be the Action Replay for the Genesis. Had to send away for it from a magazine ad and then they came out with a newer version that didn't use the same codes or something so it wasn't very useful in the long run. Of course if I'd just waited I could've gone to any store in America and bought a Game Genie instead, but I'm impatient, lol. I even went to Canada to buy a NES Game Genie back when they were banned in the US.

I only know one other person who has it. I know next to nothing about it. I'd like to know how it was circulated.
EDIT: Doing a little searching, I read where someone said they mailed in some candy wrappers for a Lost World game or demo (forget which they said) but got SSv2 instead.

Oddly enough, Darkwing Duck for the Turbografx-16, which was given to me by a friend along with a healthy handful of other TG-16 games.
The rarest I think I own is Guilty Gear NTSC (PSX) (7 on the digitpress guide).
But since I'm based in Europe my collection is mostly PAL where it's generally harder to find real rarity indicators (apart what you see in the wild, which could be anecdotic evidence).
EDIT: Hmm, although it's rated 7 on the digitpress guide there are 3 copies to be found on Ebay at the moment, so that could indicate it's not really that rare :roll:

I have a couple real rarities, but these 2 prototypes are my rarest. I know for certain I am the only person who has either of these 2 titles. They are both equally rare. They are both from the same company, which also produced Word Safari: Friendship Totems and Math Patrol: The Kleptoid Threat, the 2 rarest GBA games.

My megaman X Gamecube controller by nubytech is easily the rarest thing I own. Sadly, it's got a lot of wear from the previous owner as the rubber on the main analog stick is completely gone and the A button sticks quite a lot, but it did ship with the original half-orb packaging.

Id have to say my Mogul Maniac Romox cart for the atari 8 bit. Simply put I have not seen another one anywhere in cart form. Isnt a prototype but its still a neat addition to a collection.
I wish I could get one of those education master carts that dorsett made. I lost out a group lot of 2 of them ( of the same I take it with the same model # on the label) and Supersalt. I figured after 90.00 It was getting out of control. Supersalt I could get online for 35.00 and the dorsett carts were unknowns.

My rarest I would gather by the Price I paid is Bubble Bobble 2 for NES, though I would think maybe due to not many of Jaguar anything being published I am hoping my Cybermorph and Iron Soldier is up there too.

I think the rarest videogame in my collection is Fatal Fury Special (PAL) for the MegaCD (SegaCD). But I knew the game was a rarity until a week ago :roll:

A DS game, Emily the Strange, No. 4 of 200 LE copies or Knights of the Temple 2 UK PAL version for the PS2. Have Suikoden, Lunar and other Zelda rare stuff worth more but they're the rarest.

Also rare to me is a game that you can't get, funnily enough because it's rare. Panzar Dragoon, Suikoden 2 and Little Samson are uncommon games (IMHO), not rare. If you wanted one you could go buy one from eBay, it would just be expensive. Hoppie for the Playstation 2 is a rare game for example, mainly because, again, if you wanted one you can't buy one, because it's rare.
Probably the Entex Adventurevision or the two sealed AV games I own.

Not completely sure, but my US Sengoku 2 and 3 for Neo Geo AES are pretty rare. I know Sengoku 3 usually doesn't pop up at auction much for a legit US or Euro copy. Usually just conversions.

As odd as it may be since it's a modern system ever since I picked up the retail boxed version of Retro City Rampage DX that would have to be the rarest game item since there was only a 2000 game print run of that one on PS4.

It's a great game, and I encourage anyone with a Wii to pick it up on the Virtual Console. Sure beats the ridiculous amount people are charging for the original MD cart.

Well I got that rare PS4 game a few months ago which had a run of 2000, but I beat that out a couple weeks ago.
Introducing Bally/Midway 1983 - Gold Ball (Pinball machine.) 1750 of them were only made back in 1983 and that's it, it was a rare low run table and also unique as it was the only game to ever have an actual gold (brass) ball to have in play when you meet certain conditions. Initially a random crap shoot of 1-15% of it ever appearing within any games played, but if you have the service kid (or Home ROM v12 which includes that, which is what I have) you can set it to be a regularly earned ball during play. I guess Bally thought better of having a triple score ball almost never show up as it was infuriating to enough people so they made it a skill thing to earn.
I entered the big leagues on this jumping from home pins or plastic tabletop funk but I've wanted one for years, played it at the Louisville Arcade Expo and I had to have it.

Yeah, I don't know exactly how rare it is, but it is definitely getting up there in price. I got a good deal on mine in a lot I got frim a local store that no longer exists. It contained, all complete in box with all of the plastic and paperwork, a Mega Drive 1, Mega CD2, Super 32X, a spare 3 button controller, Sega Tap, and Sega Modem, and about 30 games (all CIB with posters and such) that included some good ones like Pulseman, Sailor Moon, Bare Knuckle II and III, Rent a Hero, Gunstar Heroes, and more, for $200. At the time I wasn't sure if it was a good deal, as I hadn't priced it all out, and didn't know I would be buying it until I got there and stumbled across the huge box it was all sitting in. Turns out I did pretty well!

I don't have anything SUPER rare, but I do have a few things that are pretty rare.
My Casio Loopy, CIB and in near mint condition would definitely be up there. (It was 400 bucks, so it better be pretty rare!)

I also have a Vectrex, but it's loose. Plumbers Don't Wear Ties for 3DO, Dig Dug for Intellivision, Mean 18 Golf for Atari 7800, and Golden Axe Warrior for Sega Master System are some games I can think of.
Golden Axe Warrior was 16 bucks, boxed with manual, on July 12, 2009 at a going out of business sale, my favorite video game store closed. I would say easily half or more of my pre-PS1 games are from that store.
It's probably not the rarest thing ever, but I also have Zelda Collector's Edition for Gamecube still in its original shrink wrap! I received it for Christmas in 2003, and it's never been opened. 5,202 days in its original wrapper.
